Nurse-led care, planned around you
Dementia Care
For someone living with dementia, familiar surroundings and patient, consistent support can be important parts of daily life. We plan home care around the person, their preferences and the routines that help them feel comfortable.
Support for someone living with dementia
Familiar routines, a calm approach and support that respects the person's choices can help make everyday care feel more manageable. We discuss the person's abilities, communication preferences and support needs with them and, where appropriate, the people involved in their care. Suitability is considered individually and reviewed as needs change.
Practical, person-centred support
The assessed care plan may include support with:
Personal care and daily tasks
Patient prompts and respectful assistance with washing, dressing and routines, allowing time and offering choices where appropriate.
Communication and reassurance
Using a calm manner, simple explanations and one step at a time, based on what the person finds familiar and helpful.
Meals and drinks
Encouragement and agreed assistance with meals and hydration, while noticing and reporting relevant changes or concerns.
Meaningful activities
Conversation and activities linked to the person's interests, relationships and abilities, where appropriate during the planned visit.
Medication support
Support only at the assessed and agreed level, following current care and medication instructions and the carer's competence.
Everyday safety
Following agreed risk assessments and keeping the areas used during the visit clear and comfortable.
All support is subject to assessment, the agreed care plan and our carers' training and competence. Additional activities must fit safely within the agreed visit time without displacing essential care.
A consistent, respectful approach
Learn what matters to the person
We ask about life history, preferences, communication and familiar routines that can help carers provide considerate support.
Offer time and choices
Care is planned around the person's wishes and abilities. If they decline a task, carers follow the agreed guidance, record relevant information and report concerns appropriately.
Work with family appropriately
Family or representatives can share useful information and receive appropriate updates when this is consistent with consent, confidentiality and the person's circumstances.
Planning dementia support
Start with the person's day
Tell us about routines, communication, interests, the support already in place and what the person wants help with.
Discuss needs and risks
We assess the support requested and relevant risks with the person and, where appropriate, their representative.
Agree clear care guidance
The care plan records agreed tasks, preferences, contacts and how carers should respond to concerns or changes during a visit.
Review when things change
We review the plan when the person's needs, risks or circumstances change. Health or clinical concerns should also be discussed with the appropriate health professional.
Care records and communication
Carers record the support provided, relevant changes, refusals and concerns in line with the care plan. Medication support is documented in the appropriate record. Significant concerns are escalated through the agreed procedures, and information is shared with family or representatives where appropriate and lawful.

Can someone living with dementia receive home care?
We can discuss the person's needs and preferences and assess whether the requested support is suitable for our service. The care plan and staffing arrangements must be appropriate to the person's assessed needs and risks.
How do carers support communication?
We discuss what works for the individual. This may include speaking calmly, using clear short explanations, allowing time to respond and offering simple choices, in line with the person's care plan.
Can family help create the care plan?
Where appropriate, family members or an authorised representative can share routines and preferences. We take account of the person's wishes, consent, confidentiality and any relevant authority.
What if the person declines care during a visit?
Carers follow the agreed guidance, respond respectfully and do not force care. Relevant refusals or concerns are recorded and reported in line with the care plan and our procedures.
Can carers provide medical treatment for dementia?
Our home care supports agreed daily care needs. It does not replace diagnosis, treatment or clinical advice from the person's health professionals. We report relevant concerns through the agreed care arrangements.
Can support continue as needs change?
We can review the care plan with the person and, where appropriate, their representative. If needs or risks change, we discuss whether the existing arrangements remain suitable or need to be revised.
How much does dementia care cost?
The cost depends on the assessed support, visit schedule and staffing arrangements. We discuss the proposed care and charges before visits begin.
